We generally get this problem when attempting to drop a database or during an upgrade/installation process. The error message would look like this:

Use Case 1

Here, the user has an issue while trying to drop the database with drop database foo;.

The customer has a SQL Server 2012 database running on Amazon EC2 and has created a user with the ability to add, update, and delete databases. The new user has the dbcreator server role.

The user can log in remotely and successfully execute the command create database foo;. However, when the user tries to drop the database using drop database foo; again, the operation fails with the following error:

Warning: Fatal error 615 occurred at Feb  1 2014  5:15PM.
   Note the error and time, and contact your system administrator.
ErrorCode: 21

Solution: In this case, we have to set the AutoClose option for the database to False. Check the model database to ensure it is not set to auto-close.

Use Case 2

In this case, when the customer tried to upgrade/install application software, the installer returns the below error:

"Exception: Warning: Fatal error 615 occurred."

Solution: The error occurs as the database user does not have the necessary permissions set in the database. For a successful upgrade/installation, database privileges must be upgraded. The correct permissions needed to successfully upgrade/install each application differ from each other. we have to make sure everything is set correctly according to each application.
